- Supporting individuals in understanding and negotiating lease agreements.
- Facilitating introductions and relationship building with prospective landlords.
- Promoting cultural needs and understandings with landlords and property managers.
- Assisting in securing funding for deposits.
- Helping individuals plan and organize their move.
- Researching potential housing options.
- Contacting potential housing providers on behalf of the individual.
- Identifying resources to cover deposits or necessary household items.
- Finding resources to assist with moving expenses.
- Completing housing applications for the individual.
- Assisting with record expungement or obtaining reasonable accommodations.
- Identifying services and benefits to support individuals facing housing instability.
- Ensuring the new living space is safe and ready for occupancy.
- Arranging necessary adaptive accommodations for housing.
- Organizing assistive technology as needed by the individual.
Services Not Covered:
- Room and board costs (including moving expenses).
- Deposit amounts.
- Food expenses.
- Furnishings.
- Rent payments.
- Utility costs.
- Missed appointments.
- Travel time for staff when the individual is not present or when housing discussions do not occur.
- Services provided in group settings.
- Transition services from institutions if the individual passes away before discharge or if services exceed 180 days.
- Physical tours of housing located outside of Minnesota.
- Housing transition services that duplicate other available assistance.
