Housing Consultation Services

Services We Provide:

• Assisting individuals in obtaining the necessary documentation for Housing Stabilization Services eligibility.
• Creating a Housing-Focused Person-Centered Plan based on the results of assessments.
• Helping individuals identify their strengths, needs, and preferences in housing, including cultural considerations.
• Supporting informed decision-making regarding housing transitions or sustaining providers.
• Providing information about additional resources that support non-housing related goals identified during the planning process.
• Coordinating with other service providers involved with the individual.
• Educating individuals about their rights to privacy and the appeals process.
• Updating the person-centered plan annually in relation to housing.

Services Not Covered:

• Housing-Focused Person-Centered Plans for individuals with Medicaid-funded case managers or MSHO/MSC+ care coordinators, or those deemed ineligible for Housing Stabilization Services by DHS.
• Missed appointments.
• Gathering documentation and verification after the completion of the Housing-Focused Person-Centered Plan.
• Conducting assessments.
